For the protection of patients you will be caring for as well as your own protection from infectious diseases, the Academic Policy requires that all students have current immunizations for measles, mumps, rubella, tetanus, hepatitis B and either proof of having chickenpox or a chickenpox titer, and tuberculosis screening. Your family physician, nurse practitioner or health department official must complete the enclosed form or you must attach copies of your immunization records. Please turn all immunization documentation in to Academic Affairs, Room 123, on or before August 16th. If you do not have a record of your immunizations and cannot obtain them from a previous MD/FNP or school, you will need to receive the immunizations again or have a titer drawn to verify exposure. There will be no exceptions made so please begin looking for your records or taking proper measures to verify immunological protection. This documentation is due the first day of orientation and lack thereof will jeopardize your ability to begin courses in a timely manner.
